How do I get my brother to show he cares?

For the past thirty years, my brother has not shown a single bit of affection for me. When I was diagnosed with HIV at age eleven, he was just making sure his grades weren't slipping. When I was 16 and locked in an insane asylum for 3 months, he didn't visit me once, wereas our friends did, even the ones who couldn't care less any other time. When I was 18 and in the hospital for 6 months due to treatment on my liver cancer, he never stopped in once, and when I got home, he didn't care. Now he's saying that he loves me with all of his heart, but with all of those times he wasn't there, I can't believe what I hear. Is there any way for me to be sure he really loves me? Because he's never said it before, and it's really suspicious to me...
